Job Purpose:

The Bilingual Flex Housing Specialist is responsible for conducting initial telephonic assessments of all new housing referrals that are received from our contracted Accountable Care Organizations (ACOs) for housing support. The Housing Specialist is a member of the Community Supports Department and assists in related activities as appropriate and as time allows.  This position will help facilitate housing support services as a recognized social determinant of health to advance the integration of health care and community-based supports.

Duties and Responsibilities:

Essential Job Functions:

  • Receive new referrals for housing flex services and contact ACO member to schedule an assessment of housing support needs in the community.
  • Conduct a thorough telephonic assessment consistent with existing screening tools to determine unmet needs, availability of formal and informal supports, program eligibility, barriers to participation, and overall willingness to receive and engage in services.
  • For members who do not wish to proceed with the housing support services, update tracking record, WILD database and provide disposition information to Manager and ACO referral source.
  • Assists the member/enrollee in identifying available public, private and informal housing supports that will meet their needs and preferences.
  • Timely complete data input in appropriate databases, which includes demographic data and program/services documentation. Finds resources and services in the community that support members/enrollees, independence, wellness, and goals of recovery.
  • Identify Housing Barriers and provide Peer Counseling, Housing Expertise, and Resources
  • Provide information about the different types of housing available in Massachusetts; public, private, subsidized, project-based, etc. Assist with State, Federal, and specialized housing registries.
  • Completion of CHAMP (Common Housing Application for Massachusetts Housing Program) online application
  • Completion of other paper and online applications **As applicable per member – each housing program has different qualifying criteria and CORE documents are necessary to obtain housing
  • Assist Member with CORE documents obtainment
  • Provide member with individual advocacy and support including answering member questions, addressing concerns, explaining qualifying criteria, and general guidance throughout the process
  • Educates members/enrollees on community related programs and activities.
  • Attends and participates in all mandatory meetings and trainings, including Interdisciplinary Care Team meetings with the ACOs.
  • Maintain knowledge of, and carefully follow, all applicable federal and state compliance requirements and regulations.
  • Committed to ensuring the member’s/enrollee’s voice, needs and preferences are supported to remain independent in the community. Promotes and values members dignity of risk and right to fail in a person-centered model of care.
  • Works as an effective team member in a collaborative manner to ensure continuity of adequate and appropriate information sharing.
  • Other duties as assigned.

  • Maintain updated computer technology competencies i.e., Microsoft Word, Excel, Outlook, and WILD

  • Bilingual in English and Spanish required.
  • Bachelor’s Degree in Human Services or related field desired. May be substituted with equivalent experience living with a disability
  • Three to five years of experience in the human service field and experience in coordinating services for one or more of the target populations.
  • Experience working with individuals with chronic health conditions and physical, developmental, and mental disabilities.
  • Disability and cultural competence in mental health stigma and barriers to independence.
  • Ability to travel independently & reliable transportation required.
  • Excellent computer skills required.
  • Strong written and verbal skills. Well organized, ability to multitask and strong attention to detail.
  • An enthusiastic team player who adapts well to on-going change and welcomes new challenges.
